Alar Ligament Test
This is a test for upper cervical spine instability of the Alar ligament. To execute the test, first position the patient in sitting and assess for resting symptoms. Next, locate the spinous process of C2 utilizing a pincher grasp. Once C2 is identified and appropriately palpated, next sideflex or rotate the head while assessing the coupled movement of the C2 spinous process contralterally during relative sideflexion and/or rotation. A positive test for instability is the lack of coupled C2 motion during sidebending and/or rotation.
This test has yet to be investigated for it’s clinical utility
